ANTICHOLINERGIC DRUGS

Acetaminophen, Diphenhydramine falls under ANTICHOLINERGIC DRUGS — the group Iboga interacts with.
In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this pairing. Tylenol PM combines acetaminophen with diphenhydramine, an antihistamine that works partly by blocking acetylcholine, the messenger that helps you feel alert. Lab studies hint that Iboga may raise acetylcholine levels, which could push against diphenhydramine and make it work a little less well. In real life that might mean the sleepy, drying effect feels weaker. Keep in mind this is theoretical, drawn only from test-tube work, and ibogaine itself didn't show this effect.
Honestly, the bigger concern is Iboga itself, which can cause dangerous heart rhythm problems and is illegal in the U.S. Please don't combine these without talking to your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS
2 of 2Acetaminophen, Diphenhydramine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to know with Tylenol PM. The diphenhydramine (the sleep-aid antihistamine) part can mildly block an enzyme called CYP2D6, and that same enzyme is what your body uses to break down ibogaine, the active compound in Iboga. So in theory, taking them together could slow ibogaine's clearance and let it build up higher than expected. One small study using a different CYP2D6 blocker saw roughly double the ibogaine exposure.
In plain terms, the worry is more ibogaine effect, not more Tylenol PM effect. That matters because ibogaine itself can trigger dangerous heart rhythm problems. Iboga isn't something we recommend or that's even legal here, so please don't combine these. Talk with your pharmacist or doctor if you have questions.
