Berotralstat Hydrochloride falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.

Here's the gist for someone taking Orladeyo (berotralstat) to prevent hereditary angioedema attacks. Berotralstat itself can stretch out the heart's electrical reset, what doctors call the QT interval, especially at higher doses. Iboga (through ibogaine) has been linked in case reports to the very same effect. Stack the two and you could nudge the QT longer than either alone, which in rare cases can trigger dangerous, fast heart rhythms, dizziness, fainting, or worse.

This is a theoretical, additive concern based on anecdotal reports, not hard proof, so I'm not trying to scare you. But ibogaine carries serious, even fatal, heart risks on its own and isn't something I'd recommend. Please talk with your pharmacist or doctor before mixing anything with Orladeyo.

Berotralstat Hydrochloride also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand about this pairing: the concern runs toward the Iboga side, not the other way around. Berotralstat (Orladeyo), the medicine you take to prevent hereditary angioedema attacks, is known to block the CYP2D6 enzyme. Ibogaine, the active compound in Iboga, is broken down by that same enzyme. So if you slowed that enzyme down with berotralstat, ibogaine could pile up in your system, possibly doubling its levels based on a small study using a different blocker.

That matters because ibogaine can trigger dangerous heart rhythm changes on its own, and higher levels mean higher risk. Honestly though, Iboga is illegal in the U.S. and unsafe outside of research, so I wouldn't use it at all. Please talk with your doctor or pharmacist before changing anything.
