Ibogaine, a constituent in iboga, has serotonergic effects. Theoretically, combining serotonergic drugs with iboga might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using iboga with serotonergic drugs.

Here's the gist for this combo. Your Ricobid product pairs chlorpheniramine (an antihistamine for sneezing and runny nose) with phenylephrine (a decongestant). Chlorpheniramine has mild effects on serotonin, and Iboga (through ibogaine) also nudges serotonin upward. On paper, stacking the two could push serotonin too high. The worry would be serotonin syndrome: agitation, a racing heart, sweating, shivering, muscle twitching, or confusion.
I want to be honest about the strength here. This is a theoretical concern based on how these substances behave, not on solid case reports. That said, Iboga carries its own serious heart rhythm risks and is illegal in the U.S., so I'd steer clear of it entirely. Please check with your pharmacist or doctor before mixing anything.

ANTICHOLINERGIC DRUGS

In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.

Here's the gist for your Ricobid product. The chlorpheniramine part is an antihistamine that works partly by blocking acetylcholine, the body's 'rest and digest' messenger. Some lab studies suggest Iboga might raise acetylcholine levels by slowing the enzyme that breaks it down. In theory, that extra acetylcholine could push back against chlorpheniramine and make it work a little less well, so things like a runny nose, sneezing, or itchy eyes might not be as controlled.
I want to be honest though: this is only test-tube data, and interestingly the main active compound ibogaine didn't even show this effect. So it's a weak, theoretical concern. Far more importantly, Iboga itself carries serious heart rhythm dangers and is illegal here. Please talk with your pharmacist or doctor before combining anything.

CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS
3 of 3Chlorpheniramine Tannate, Phenylephrine Tannate also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the situation with your cold and allergy combo (chlorpheniramine, an antihistamine, plus phenylephrine, a decongestant). The chlorpheniramine piece can gently block a liver enzyme called CYP2D6, which happens to be the same enzyme your body uses to clear ibogaine, the active compound in Iboga. When that pathway is slowed, ibogaine and its breakdown product can build up. In one study, a CYP2D6 blocker roughly doubled ibogaine exposure.
The real concern here isn't your cold medicine, it's the Iboga itself. Ibogaine is illegal in the U.S. and can cause dangerous heart rhythm problems and even death. Higher ibogaine levels mean higher risk. Please don't use Iboga, and talk with your pharmacist or doctor if you have any questions.
