Chlorpromazine falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.

Here's the concern with putting these two together. Chlorpromazine (Thorazine) is already known to stretch out what's called the QT interval, which is a measure of your heart's electrical rhythm. Iboga, through its main compound ibogaine, has been linked in several real-world case reports to the very same effect. Stack them and the worry is that those changes add up, which in rare cases can trigger a dangerous, irregular heartbeat.

I'll be honest, this is mostly based on scattered case reports rather than firm studies, so the evidence is thin. But ibogaine carries real, documented heart risks (and it's illegal in the U.S.), so I wouldn't take chances here. Please talk with your doctor or pharmacist before combining anything with your chlorpromazine.

Theoretically, iboga might decrease the effectiveness of anticholinergic drugs.
In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.

Here's the idea behind this pairing. Chlorpromazine has some anticholinergic activity, meaning part of how it works is by dialing down the nervous system messenger acetylcholine. Lab (test tube) studies suggest Iboga might do the opposite by raising acetylcholine levels, which could, in theory, work against chlorpromazine and blunt some of its effects. Interestingly, the main alkaloid ibogaine itself didn't show this effect in those studies.

This is very early, theoretical evidence only, so I wouldn't lose sleep over it. Honestly, my bigger concern is that both Iboga and chlorpromazine can affect heart rhythm and cause serious problems on their own. Iboga is illegal and genuinely risky, so please don't combine them, and talk with your doctor or pharmacist first.

What Is Chlorpromazine Used For?

Brand names: Thorazine, Thorazine Spansules

Chlorpromazine is used to manage psychotic disorders and treat schizophrenia. It also controls nausea and vomiting, relieves restlessness and apprehension before surgery, and controls the manic phase of manic-depressive illness. Other uses include relief of intractable hiccups, treatment of acute intermittent porphyria, and use as an adjunct in tetanus treatment.

In children ages 1 to 12, it is used for severe behavioral problems marked by explosive or combative behavior, and for short-term treatment of hyperactive children with conduct disorders. The oral forms are also approved for the management of manifestations of psychotic disorders broadly, in addition to the uses shared with the injection form.
