Why this applies to Darunavir, Ritonavir
Darunavir, Ritonavir falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Iboga interacts with.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the key thing about this combo: it's really the Iboga that gets affected, not so much your HIV medicine. Ritonavir (often paired with darunavir as a booster) is a strong blocker of a liver enzyme called CYP2D6, and that same enzyme is what your body uses to clear ibogaine, the active part of Iboga. When ritonavir slows that pathway down, ibogaine can pile up. One study using a milder blocker roughly doubled ibogaine exposure, so a strong booster like ritonavir could push those levels even higher.
Why that matters: ibogaine can cause dangerous heart rhythm changes on its own, and higher levels raise that risk. On top of that, ibogaine is illegal in the U.S. and genuinely unsafe outside strict research. Please don't combine these, and talk with your doctor or pharmacist first.
