Halofantrine falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.

Here's the concern with pairing these two. Halofantrine is a malaria medicine that's well known for stretching out the heart's electrical cycle, something doctors call QT prolongation. Iboga (through its compound ibogaine) appears to do the very same thing, based on a number of case reports. Put them together and those effects can stack up.

When the QT interval gets too long, it can trigger a dangerous, fast heart rhythm that may cause fainting, a racing or fluttering feeling in the chest, or in rare cases something life-threatening. This isn't about one drug breaking down the other, it's two separate heart effects adding together. Honestly, Iboga carries serious heart risks on its own and is illegal in the U.S., so please talk with your doctor or pharmacist before going anywhere near this combination.

Halofantrine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand: Halofantrine slows down the CYP2D6 enzyme, and your body uses that same enzyme to clear ibogaine (the active compound in Iboga). So if these two were combined, ibogaine could pile up to higher levels than expected. A study with another CYP2D6 blocker roughly doubled ibogaine exposure, so this is a real concern, not just theory.

What worries me most is the heart. Both Iboga and Halofantrine are known to dangerously lengthen the heart's QT interval, which can trigger life-threatening rhythm problems. Stacking them, plus higher ibogaine levels, raises that risk further. Please don't combine these. Talk with your doctor or pharmacist before making any changes.
