Here's the gist for acalabrutinib (Calquence). Your body relies on an enzyme called CYP3A4 to break this cancer medicine down and clear it. Piperine, the main compound in Indian long pepper, has been shown in lab dishes to slow that same enzyme. If it does the same thing in real life, acalabrutinib could build up a bit higher in your blood, which may mean more of its known side effects like headaches, bruising or bleeding, infections, or low blood counts.

I want to be honest about the strength of this: it's theoretical, based on test-tube data, not on people taking both together. A pinch of this spice in food is unlikely to matter much, but regular concentrated supplements are a different story. Since acalabrutinib is serious therapy, please run it by your oncologist or pharmacist before mixing the two.

Here's the gist for Calquence (acalabrutinib): this is a cancer medicine that your body partly moves around using a transporter called P-glycoprotein. Piperine, the active compound in Indian long pepper, has been shown in lab studies to slow that transporter down. In theory, that could let a bit more acalabrutinib stay in your blood, which might mean stronger effects and a higher chance of side effects like bruising, bleeding, headaches, low blood counts, or infections.

The important caveat: this is based only on test-tube research, not real patients, so we genuinely don't know if it matters in everyday life. The pinch of pepper in food is unlikely to be a problem, but concentrated pippali supplements are worth mentioning. Since this is a serious cancer drug, please check with your oncologist or pharmacist before adding it.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
