Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this headache combo. The piperine in Indian long pepper can, at least in lab studies, slow down an enzyme called CYP3A4. That enzyme helps clear part of this medication, especially the butalbital (the barbiturate that relaxes you) and some of the caffeine. If that pathway is blocked, those pieces could linger a bit longer and build up.
In plain terms, you might feel more drowsy, lightheaded, foggy, or overly sedated than usual. The good news: this is only a theoretical concern based on test-tube data, not real patient reports, and a little pepper in your food is very unlikely to matter. Concentrated pippali supplements taken daily are where I'd be more cautious. If you use it regularly, just give your pharmacist or doctor a heads-up.
