Acetaminophen, Caffeine, Dihydrocodeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with this combo pill. The piperine in Indian long pepper can mildly slow down a liver enzyme called CYP3A4, and that's one of the pathways your body uses to clear dihydrocodeine and caffeine. In theory, that could let these ingredients linger a bit longer, nudging up the opioid effects like drowsiness, dizziness, constipation, or slowed breathing, plus more caffeine jitters or a restless feeling.
That said, this is a theoretical concern based only on lab studies, not real-world cases. A sprinkle of the spice in cooking is very unlikely to matter. Concentrated supplement doses are where I'd be more careful. If you take pippali regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on things.
