NyQuil is a combo product, and a couple of its ingredients (especially dextromethorphan, the cough suppressant) are partly handled by a liver enzyme called CYP3A4. The piperine in Indian long pepper can slow that enzyme down in lab studies. In theory, that could let these ingredients linger a bit longer, nudging up effects like drowsiness, dizziness, or feeling groggy. None of these are prodrugs, so blocking the enzyme would tend to raise their levels, not weaken them.

Here's the honest part: this is theoretical, based only on test-tube data, and ordinary culinary amounts of pepper are unlikely to matter much. Concentrated supplement doses are the bigger question mark. If you use long pepper regularly, just give your pharmacist a heads-up before reaching for NyQuil.
