Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Avanafil
Avanafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your body relies on an enzyme called CYP3A4 to clear Avanafil (Stendra) out of your system. The piperine in Indian long pepper may slow that enzyme down a bit, so the medication could hang around longer and reach higher levels than usual. In practice, that might mean a stronger dose effect, which could show up as headache, flushing, a stuffy nose, dizziness, or low blood pressure.
I want to be honest about the evidence here: this comes from lab (test tube) studies only, so it's more of a theoretical concern than something proven in people. A pinch of the spice in food is unlikely to cause trouble, but concentrated supplements are a bigger question. If you take it regularly, just give your pharmacist or doctor a heads-up.

From our pharmacist’s overview on the Avanafil page
“Avanafil (Stendra) treats erectile dysfunction by widening blood vessels, and I read this list of 144 supplements as one that warrants real care rather than panic. Most of the risk runs through a single liver enzyme, CYP3A4, which clears avanafil and which 121 of these ingredients can affect.”
