Ibuprofen, Hydrocodone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo pill pairs ibuprofen (an anti-inflammatory) with hydrocodone (an opioid pain reliever). The hydrocodone part is handled in the body partly by an enzyme called CYP3A4. Piperine, the active compound in Indian long pepper, has been shown in lab dish studies to slow that same enzyme down. In theory, that could let a bit more hydrocodone build up, which might mean stronger effects like extra drowsiness, dizziness, constipation, or slowed breathing.
I want to be honest with you though: this is only a theoretical concern based on test-tube research, not real-world patient cases. A pinch of this spice in food is very unlikely to matter. Concentrated supplement doses are where I'd be more cautious. If you take it regularly, just give your pharmacist or doctor a heads-up.

Here's the gist for your Ibucodone (the ibuprofen plus hydrocodone combo): the ibuprofen part already has a mild blood-thinning effect because it dampens how your platelets stick together. Indian long pepper, in lab studies, seems to do something similar (it slowed platelet clumping in a test tube). So in theory, pairing the two could nudge your bleeding risk a little higher. Think easier bruising, nosebleeds, or stomach upset, since ibuprofen can already irritate the gut.
I want to be honest though: this is based only on early lab research, not real people, so it's a soft concern rather than a red alert. A pinch of pepper in food is nothing to worry about. If you take long pepper as a regular supplement, just give your pharmacist or doctor a heads-up so they can keep an eye on things.
