Treximet combines naproxen (an NSAID pain reliever) with sumatriptan (a migraine medicine). Here the part that matters most is the naproxen. Like other NSAIDs, naproxen can already make platelets a little less sticky and can irritate the stomach lining, so it carries some bleeding risk on its own.

The concern with Indian long pepper is that, in lab (test tube) studies, its extract also kept platelets from clumping together. Stacked on top of naproxen, the worry is a slightly higher chance of bruising, nosebleeds, or stomach bleeding. This is theoretical and based only on lab work, not on people, so don't panic. A pinch of pepper in food is unlikely to matter, but concentrated pippali supplements are worth mentioning to your pharmacist or doctor.
