Relugolix, Estradiol, Norethindrone Acetate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Myfembree (relugolix, estradiol, and norethindrone). All three pieces of this combo pill are handled, at least in part, by a liver enzyme called CYP3A4. The piperine in Indian long pepper can gently slow that enzyme down in lab studies, so in theory the levels of these hormones could creep up a bit and bring on more side effects, things like nausea, headache, breast tenderness, mood changes, bloating, or a slightly higher concern about blood clots.
I want to be honest though: this is based only on test-tube research, not real people taking the two together, so it's more of a caution than a proven problem. An occasional sprinkle in food likely won't matter, but a daily concentrated supplement is worth mentioning. Please loop in your pharmacist or doctor before starting it.

P-GLYCOPROTEIN SUBSTRATES

Here's the simple version: Myfembree contains relugolix, which relies on a transporter called P-glycoprotein (P-gp) to move it through your body. The piperine in Indian long pepper may block that transporter, at least based on lab studies. If P-gp slows down, more relugolix could stay in your blood than usual. In theory that might mean stronger effects or more side effects, things like hot flashes, headaches, mood changes, or unusual bleeding.
I want to be honest about how soft this evidence is. This is a theoretical concern from test-tube research, not from real people, and a sprinkle of pepper in cooking is unlikely to matter. Concentrated pippali supplements are the bigger question. If you take it regularly, just give your pharmacist or doctor a heads-up before mixing the two.
