Tramadol, Acetaminophen falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Tramacet, which combines tramadol with acetaminophen. The piperine in Indian long pepper can gently slow down CYP3A4, one of the liver enzymes that helps break down tramadol. In theory, that could let a bit more tramadol linger in your system, nudging up both its pain relief and its side effects like drowsiness, dizziness, nausea, or constipation. There's also a small chance of feeling more sedated than usual.
I want to be honest though: this concern comes only from lab (test tube) research, not from real people, so it's more of a heads-up than a red alert. A pinch of this spice in cooking is very unlikely to matter. Concentrated supplement doses are where I'd pause. If you take it regularly, just give your pharmacist or doctor a quick mention.
