Here's the simple version: tramadol already calms the nervous system to ease pain, and one of its common effects is making you feel drowsy. Jamaican dogwood has its own sedative, muscle-relaxing properties from compounds in the bark. Put them together and you could feel extra sleepy, foggy, dizzy, or unsteady on your feet. In some people that added drowsiness can be enough to slow breathing or raise the risk of a fall.

I'll be honest with you, this concern is theoretical, based on how these substances behave rather than solid human studies. It isn't about tramadol building up in your body, just two calming agents stacking. Still, since Jamaican dogwood can be toxic in larger amounts, please check with your pharmacist or doctor before combining them.
