Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Japanese Persimmon interacts with.
Some anticoagulant and antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, indomethacin (Indocin), ticlopidine (Ticlid), warfarin (Coumadin), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: alteplase (tPA) is a powerful clot-busting medicine given in the hospital for emergencies like a stroke, heart attack, or a serious blood clot in the lungs. It works by breaking down clots, so anything that also thins the blood could, in theory, add to that effect and raise the risk of bleeding (bruising, oozing, or more serious internal bleeding).
The concern with Japanese persimmon leaf is purely theoretical. In a lab dish, certain leaf protein fractions showed clot-slowing activity, but this has never been shown in actual people. Eating the fruit as food is very unlikely to matter. Also keep in mind that alteplase is given in a controlled hospital setting where your team watches you closely. Still, mention any persimmon leaf supplements to your doctor or pharmacist beforehand.
