THEOPHYLLINE

Animal research suggests that Job's tears might enhance absorption of theophylline in the small intestine. Single dose and short-term oral administration of Job's tears bran ethanolic extract along with oral administration of a five-drug cocktail containing theophylline increases theophylline peak plasma concentration and area under the plasma concentration-time curve (AUC) without altering major cytochrome P450 activities in the liver. This effect has not been reported in humans.

Here's the gist with this pairing: in animal studies, an extract from Job's Tears bran appeared to help the gut soak up more theophylline, nudging its blood levels higher. It didn't change the liver enzymes that normally break theophylline down, so this looks like an absorption effect, not a metabolism one.

Why care? Theophylline is a narrow-therapeutic-index drug, meaning the gap between a helpful dose and a toxic one is small. If levels climb too high, you can see nausea, a racing or pounding heartbeat, jitteriness, trouble sleeping, or in serious cases seizures. That said, this comes only from animal research and hasn't been shown in people, so don't panic. Before adding concentrated Job's Tears supplements, check in with your pharmacist or doctor.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.

All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
