Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with kefir and Temsirolimus (Torisel). This isn't a metabolism issue, so it's not about kefir making your drug levels go up or down. Temsirolimus calms down your immune system as part of fighting kidney cancer, which means your body is less able to fend off germs. Kefir is a fermented drink that contains live, actively growing bacteria and yeast.

The concern is theoretical: putting live microbes into a body with a weakened immune defense could, in rare cases, raise the risk of an infection. This worry comes from lab-based reasoning, not from strong studies in people. Still, while you're on this medication, it's worth checking with your oncologist or pharmacist before adding kefir or other probiotic foods.

From our pharmacist’s overview on the Temsirolimus page

“Temsirolimus (Torisel), an infusion for advanced kidney cancer, deserves genuine wariness around supplements. Most interactions on record involve herbs that alter CYP3A4, the liver enzyme that clears the drug, so a capsule can quietly raise its levels and side effects or thin out its anticancer effect.”
