Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Kratom interacts with.
In vitro research suggests that kratom extract inhibits the activity of CYP1A2. In one case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited CYP1A2-mediaed metabolism of ziprasidone which consequently increased systemic exposure to serotonin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This is a multi-ingredient cold and pain product (acetaminophen for pain and fever, chlorpheniramine for sniffles, phenylephrine as a decongestant, and salicylamide for pain). The concern on file is that kratom may slow down a liver enzyme called CYP1A2, which can help clear certain medicines. In theory, that could let a drug linger a bit longer and hit harder.
For this combo, none of these ingredients lean heavily on CYP1A2, so I would not expect a big change. Still, kratom and chlorpheniramine can both cause drowsiness, and stacking them may leave you extra sleepy or foggy. Just so you know, this link is theoretical, based on lab data, not real patient cases with this product. Please run it by your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

This cold and pain combo (Rhinogesic) blends acetaminophen, the antihistamine chlorpheniramine, the decongestant phenylephrine, and salicylamide. The concern here is theoretical: lab studies suggest kratom can block a liver and gut enzyme called CYP3A4, and a few of these ingredients lean on that pathway to be cleared. If kratom slows their breakdown, levels could drift a bit higher than expected.
In plain terms, you might feel more of the usual effects, things like extra drowsiness or grogginess from the antihistamine, or a jittery, racing-heart feeling from the decongestant. Honestly, the bigger worry is kratom's own sedating, opioid-like effects stacking on top. The evidence for this exact interaction is weak, but I'd still run it past your pharmacist before combining them.

It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Let me put this one in perspective. This cold and pain combo (acetaminophen, chlorpheniramine, phenylephrine, and salicylamide) isn't a classic serotonin booster, so the serotonin syndrome warning tied to kratom really doesn't fit it well. The concern on file is mostly theoretical and based on a single case report involving very different antidepressant-type medicines.
That said, mixing kratom with this product still isn't a good idea. Both kratom and chlorpheniramine can make you drowsy, foggy, and slow to react, so together they can stack up that sleepiness. Phenylephrine can also bump up blood pressure and heart rate, which adds another reason for caution. Please check with your pharmacist or doctor before combining these, especially since kratom itself carries real risks.
