Why this applies to Aspirin, Omeprazole

Aspirin, Omeprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kratom interacts with.

Kratom might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Here's the honest picture with Yosprala, which combines aspirin with omeprazole. The concern on file is that kratom may block an enzyme called CYP3A4, which could slow how some drugs are cleared and let them build up. The good news is that neither aspirin nor omeprazole leans heavily on CYP3A4 to be broken down, so this particular pathway isn't a major route for your medication. Omeprazole is handled mostly by a different enzyme (CYP2C19), and aspirin is cleared by other means.

So this interaction is mostly theoretical for Yosprala and rests on lab and limited human data. My bigger worry is kratom itself, which acts on opioid receptors and carries real risks of dependence and harm. Please loop in your pharmacist or doctor before mixing the two.

Here's the gist for Yosprala, which pairs aspirin with omeprazole. The omeprazole part is handled in your body partly by an enzyme called CYP2C19. In a test-tube setting, kratom seems to weakly slow that same enzyme down. So in theory, kratom could let omeprazole build up a little. Omeprazole is generally well tolerated, so even if its levels nudged up, you'd most likely just see more of its usual effects, like lower stomach acid, and possibly mild things such as headache or an upset stomach.

Honestly, this is a lab-only, theoretical concern, and a minor one at that. The aspirin side isn't really affected here. Still, because kratom carries its own real risks, please loop in your pharmacist or doctor before combining them.
