Why this applies to Etravirine

Etravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kratom interacts with.

Kratom might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that kratom extract inhibits CYP3A4 enzyme activity. A pharmacokinetic study in adults shows that taking kratom tea 2 grams modestly increases the plasma concentration time-curve (AUC) and maximum concentration (Cmax) of CYP3A4 probe midazolam by 40-50% and its CYP3A4-mediated metabolites 18-27%. However, a lack of change in the half-life of midazolam and a simulation of the kratom-midazolam interaction suggest that kratom inhibits CYP3A4 enzymes in the small intestine but not the liver. Additionally, there is one case report of a 27-year-old male who died from neuroleptic malignant-like symptoms after concomitant use of kratom and quetiapine, a CYP3A4 substrate. Although it did not appear that the patient had consumed large quantities of quetiapine, postmortem plasma levels were in the lethal range, indicating possible inhibition of CYP3A4 by kratom. In another case report, a 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It is hypothesized that CYP3A4 inhibition by kratom reduced metabolism of desvenlafaxine, trazodone, and ziprasidone and consequently increased systemic exposure to serotonin.

Here's the situation with etravirine (Intelence) and kratom. Your body partly clears etravirine using a liver enzyme called CYP3A4. Lab studies suggest kratom can slow this enzyme down, mostly in the gut. If that happens, slightly more etravirine could stay in your system, which in theory might bump up side effects like nausea, rash, headache, or numbness and tingling.

I want to be honest though: this is theoretical, based mostly on test-tube work and one probe study, not on people taking etravirine. The real concern is that kratom itself carries serious risks and isn't a proven, regulated product. Since keeping your HIV treatment steady really matters, please don't add kratom without first talking to your pharmacist or doctor.

Here's the gist for your HIV medicine Etravirine (Intelence). Your body uses a liver enzyme called CYP2C19 to help break it down. In lab dish studies, kratom appears to weakly slow that same enzyme. In theory, that could let a little more etravirine build up in your blood, which might nudge up its usual side effects like rash, nausea, or headache.

I want to be honest though: this is only a theoretical concern from test-tube work, not real patients, and it's rated minor. The bigger worry is kratom itself, which acts on opioid receptors and carries real risks of dependence and harm. Please loop in your pharmacist or HIV doctor before mixing the two, since keeping your HIV treatment working right matters most.

Brand name: Intelence

Etravirine (brand name Intelence) is used to treat HIV-1 infection in people who have already received other antiretroviral (HIV) treatments — meaning it is meant for patients whose HIV has had a chance to develop resistance to earlier therapies. It is always used as part of a combination regimen with other HIV medications, never on its own.

Both the brand Intelence and generic etravirine tablets are approved for adults and for children ages 2 to under 18 who weigh at least 10 kg (about 22 pounds) and who have already been treated with other HIV drugs.
