ANTIHYPERTENSIVE DRUGS
Why this applies to Sacubitril, Valsartan
Sacubitril, Valsartan falls under ANTIHYPERTENSIVE DRUGS — the group L-citrulline interacts with.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. However, a meta-analysis of 5 small clinical studies suggests that taking L-citrulline 3-6 grams daily for 1-8 weeks does not lower blood pressure when compared with control.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Entresto (sacubitril/valsartan) is already doing real work to relax your blood vessels and lower blood pressure as part of treating heart failure. L-citrulline turns into L-arginine in your body, which can nudge your blood vessels to widen too. Stack the two together and, in theory, you could get a little extra drop in blood pressure.
I want to keep this in perspective, though. The evidence here is on the weak side. In fact, a pooled look at several small studies found L-citrulline didn't really lower blood pressure at all. So for most people this is a mild, possible concern, not a sure thing. Just watch for dizziness, lightheadedness, or feeling faint when you stand up, and check with your pharmacist or doctor before adding it.

What Is Valsartan and Sacubitril Used For?
Brand name: Entresto
Valsartan and sacubitril is used for heart failure. In adults with chronic heart failure, it lowers the risk of cardiovascular death and hospitalization for heart failure. Benefits are most clearly seen in people whose heart pumps less blood than normal (a below-normal left ventricular ejection fraction, or LVEF).
It is also used to treat symptomatic heart failure with left ventricular systolic dysfunction (the main pumping chamber not squeezing well) in children aged one year and older. This use is listed for Entresto and for the sacubitril and valsartan tablet products.
