Limosilactobacillus Reuteri and Piperacillin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Piperacillin

Piperacillin falls under ANTIBIOTIC DRUGS — the group Limosilactobacillus Reuteri interacts with.

Theoretically, taking Limosilactobacillus reuteri with antibiotic drugs might decrease the effectiveness of L. reuteri.
L. reuteri preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. reuteri preparations by at least two hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: piperacillin is a penicillin-type antibiotic, and it doesn't care whether the bacteria are the bad ones causing your infection or the friendly probiotic ones in your L. reuteri supplement. It kills bacteria broadly. So if you swallow your probiotic right alongside this antibiotic, the piperacillin can wipe out a good chunk of those live organisms before they ever get to work.

The good news is that this goes one direction only: it can weaken the probiotic, but it won't hurt how well the piperacillin fights your infection. This is a theoretical, pharmacology-based concern, not something well proven. If you want both, a common tip is to space them out by at least two hours. Your pharmacist can help you set up the timing.

From our pharmacist’s overview on the Piperacillin page

“Piperacillin (Pipracil) is a strong penicillin antibiotic, usually given by IV for serious infections, and two of the 39 supplement interactions on file are why I would read this list carefully rather than skim it. Both major-rated entries hit the kidneys.”
