Why this applies to Benzhydrocodone, Acetaminophen

Benzhydrocodone, Acetaminophen falls under CNS DEPRESSANTS — the group Mace interacts with.

Several volatile oils in mace, such as methyleugenol, isoeugenol, safrole, myristicin, 1,8-cineole, and geranyl acetate, seem to have sedative effects. Evidence from animal research suggests that methyleugenol can induce anesthesia for a similar duration as pentobarbital. Due to the sedative effects of certain mace constituents, taking medicinal amounts of mace in combination with other CNS depressants may have additive effects. However, evidence from other animal research suggests that myristicin can reduce sleeping time in rats pretreated with phenobarbital. Until more is known, use medicinal amounts of mace cautiously in combination with CNS depressants. Some CNS depressants include clonazepam (Klonopin), lorazepam (Ativan), phenobarbital (Donnatal), zolpidem (Ambien), and others.

Apadaz pairs a strong opioid (benzhydrocodone, which your body turns into hydrocodone) with acetaminophen. Like other opioids, it already slows down your brain and breathing and can leave you drowsy, foggy, or lightheaded. The concern here is simple: some of the aromatic oils in mace appear to have their own mild calming, sedative-type effects, so layering concentrated mace on top of this medicine could add up and make you sleepier than expected.

Keep this in perspective. This caution comes mostly from animal and lab studies, not solid human research, and the signal is mixed. Mace used as a normal cooking spice is very unlikely to cause a problem. The worry is really about concentrated mace supplements. If you're considering one while taking Apadaz, run it by your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Benzhydrocodone, Acetaminophen also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Animal research suggests that intraperitoneal injections of myristicin, a constituent of mace, can induce cytochrome P450 1A2 (CYP1A2) enzyme system. Theoretically, concomitant use of mace with drugs metabolized by CYP1A2 may increase the clearance of these drugs and reduce their effects. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the thing to know about Apadaz: the benzhydrocodone part is actually a prodrug. Your body has to convert it into hydrocodone before it can ease your pain. The interaction we have on file involves an enzyme called CYP1A2, which a compound in mace (myristicin) may speed up. If that enzyme runs faster, it could clear the active painkiller out of your system a bit quicker, possibly making it less effective at controlling your pain.

I want to be honest though: this is only a theory based on animal lab work, not human studies, and the spice amount you'd sprinkle in food is very unlikely to do much. Concentrated mace supplements are a different story. If you're taking those, just run it by your pharmacist or doctor before mixing the two.

Mace is the lacy red covering of the nutmeg seed and comes from the same tree as nutmeg. It is mostly used as a cooking spice and in traditional medicine for digestion, but there is little solid human research to support its health uses. In normal food amounts it is generally safe, but large doses can be toxic, so use it as a spice and check with your pharmacist before taking concentrated supplements.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
