Why this applies to Verapamil

Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Mace interacts with.

Animal research suggests that intraperitoneal injections of myristicin, a constituent of mace, can induce cytochrome P450 1A2 (CYP1A2) enzyme system. Theoretically, concomitant use of mace with drugs metabolized by CYP1A2 may increase the clearance of these drugs and reduce their effects. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the idea behind pairing mace with your verapamil. A compound in mace called myristicin may speed up a liver enzyme (CYP1A2), and verapamil is partly handled by that enzyme. In theory, revving up that enzyme could clear verapamil out of your body a little faster, which might make it work less well. For a heart and blood pressure medicine, that could mean your blood pressure creeps back up, more chest pain (angina), or less control over your heart rate.

The good news: this is only a theory based on animal research, not human studies, and using mace as a normal cooking spice is very unlikely to cause any problem. The caution is really about concentrated mace supplements. If you're considering one, check with your pharmacist or doctor first.

Mace is the lacy red covering of the nutmeg seed and comes from the same tree as nutmeg. It is mostly used as a cooking spice and in traditional medicine for digestion, but there is little solid human research to support its health uses. In normal food amounts it is generally safe, but large doses can be toxic, so use it as a spice and check with your pharmacist before taking concentrated supplements.

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
