Acetohexamide falls under ANTIDIABETES DRUGS — the group Maqui interacts with.

Theoretically, maqui fruit extract may increase the risk of hypoglycemia when used with antidiabetes drugs.
Animal research shows that maqui berry reduces fasting blood glucose. However, maqui berry extract taken orally, either as a single dose or daily for 3 months, does not decrease fasting blood glucose in prediabetic patients. It is unknown whether it might reduce blood glucose levels in patients with diabetes.

Here's the gist for you. Acetohexamide is an older sulfonylurea diabetes pill that works by nudging your pancreas to release more insulin, which lowers your blood sugar. Maqui berry, in lab and animal studies, has also shown a mild blood-sugar-lowering effect. So the concern is simple math: if both push in the same direction, your sugar could theoretically dip too low (hypoglycemia).

Honestly, the evidence here is thin. In actual human studies, maqui did not lower fasting blood sugar in prediabetic folks, and we don't have good data in people with diabetes. So this is more of a theoretical caution than a proven problem. If you do try a concentrated maqui supplement, just keep an eye out for shakiness, sweating, or dizziness, and check in with your pharmacist or doctor first.
