Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Methoxylated Flavones interacts with.

Some in vitro evidence suggests that methoxylated flavones might induce CYP1A2, possibly by increasing gene transcription. However, other in vitro research has not shown this effect. So far this interaction has not been reported in humans. Theoretically, concurrent use of methoxylated flavones and drugs metabolized by CYP1A2 might increase drug metabolism, decrease serum levels, and reduce effectiveness.
Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the short version: this is a very theoretical concern. In some lab dishes, methoxylated flavones from citrus peel seemed to speed up an enzyme called CYP1A2, but other lab studies didn't, and it has never actually been reported in people taking these combination cold products.

If that enzyme were sped up, it could in theory clear some medicines a bit faster and make them work less well. For this particular product, that mostly matters for the cough suppressant and decongestant pieces, so you might notice slightly less relief, like a cough or stuffiness that isn't as well controlled. None of this is dramatic, and ordinary citrus or food amounts are very unlikely to cause trouble. Still, if you take concentrated supplements regularly, it's worth a quick chat with your pharmacist.

In vitro evidence suggests that methoxylated flavones might inhibit cytochrome P450 3A4 (CYP3A4). This effect seems to be structure-dependent, and does not occur with all methoxylated flavones. So far this interaction has not been reported in humans. Theoretically, concurrent use of certain methoxylated flavones with drugs metabolized by CYP3A4 might result in increased drug levels and an increased risk for adverse effects.
Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

These citrus-peel compounds, in test-tube studies only, can slow down a liver enzyme called CYP3A4. In this combo cold product, that mostly matters for the dextromethorphan (the cough suppressant) and to a lesser degree the acetaminophen and pseudoephedrine. If that enzyme is blocked, dextromethorphan could build up a bit more, which might mean more drowsiness, dizziness, or a foggy feeling.

Here's the reassuring part: this has only been seen in the lab, never reported in actual people, and it doesn't happen with every flavone. A little citrus in your food won't be an issue. Concentrated supplements are the real question, so if you take one regularly, just run it by your pharmacist or doctor first.
