Why this applies to Erythromycin

Erythromycin falls under P-GLYCOPROTEIN SUBSTRATES — the group Methoxylated Flavones interacts with.

In vitro evidence shows that some methoxylated flavones including tangeretin, nobiletin, and heptamethoxyflavone, inhibit P-glycoprotein. Theoretically, these methoxylated flavones might increase absorption and blood levels of drugs that are transported by P-glycoprotein.
Some of these drugs include some chemotherapeutic agents (daunorubicin, docetaxel, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the short version: erythromycin gets shuttled around your body by a transport protein called P-glycoprotein, which helps move it in and out of cells. Certain citrus-peel compounds in methoxylated flavones (like nobiletin and tangeretin) have been shown in lab dishes to slow that pump down. In theory, that could let a bit more erythromycin get absorbed and build up in your blood.

If that happened, you might notice more of erythromycin's usual side effects, such as nausea, stomach cramping, or diarrhea. The thing to keep in mind is that this is theoretical, based only on test-tube work, not real patients. A splash of citrus in food is very unlikely to matter; concentrated supplements are the bigger question mark. If you're taking both, just run it by your pharmacist or doctor first.

Erythromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence suggests that methoxylated flavones might inhibit cytochrome P450 3A4 (CYP3A4). This effect seems to be structure-dependent, and does not occur with all methoxylated flavones. So far this interaction has not been reported in humans. Theoretically, concurrent use of certain methoxylated flavones with drugs metabolized by CYP3A4 might result in increased drug levels and an increased risk for adverse effects.
Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist: your body clears erythromycin partly through a liver enzyme called CYP3A4. Some methoxylated flavones from citrus peel may slow that enzyme down in lab tests. If they do, erythromycin could clear more slowly and build up a bit, which might nudge up the usual side effects like nausea, stomach cramps, diarrhea, or in rare cases heart rhythm changes (QT prolongation).

That said, I don't want to alarm you. This is only seen in test-tube and animal studies, it hasn't been reported in real people, and not all of these flavones do it. Everyday citrus in food is very unlikely to matter; concentrated supplements are the bigger question mark. Just let your pharmacist or doctor know before you combine them.

What Is Erythromycin Used For?

Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
