Why this applies to Verapamil

Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Methoxylated Flavones interacts with.

Some in vitro evidence suggests that methoxylated flavones might induce CYP1A2, possibly by increasing gene transcription. However, other in vitro research has not shown this effect. So far this interaction has not been reported in humans. Theoretically, concurrent use of methoxylated flavones and drugs metabolized by CYP1A2 might increase drug metabolism, decrease serum levels, and reduce effectiveness.
Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist for verapamil and these citrus-peel flavones (like nobiletin). In a few lab dish studies, these compounds appeared to speed up an enzyme called CYP1A2, which helps your body break down certain medications. Theoretically, if that enzyme works faster, verapamil could be cleared a bit quicker, leaving lower levels in your blood and possibly making it less effective at controlling your blood pressure, chest pain, or heart rate.

That said, I want to keep this in perspective. This is theoretical, drawn only from test-tube work, and it has never actually been reported in people. Some lab studies didn't even show the effect. Everyday citrus in your diet is very unlikely to matter. If you're considering a concentrated supplement, just run it by your pharmacist or doctor first so they can keep an eye on your numbers.

P-GLYCOPROTEIN SUBSTRATES

Verapamil also falls under P-GLYCOPROTEIN SUBSTRATES.

In vitro evidence shows that some methoxylated flavones including tangeretin, nobiletin, and heptamethoxyflavone, inhibit P-glycoprotein. Theoretically, these methoxylated flavones might increase absorption and blood levels of drugs that are transported by P-glycoprotein.
Some of these drugs include some chemotherapeutic agents (daunorubicin, docetaxel, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist: certain methoxylated flavones from citrus peel (like nobiletin and tangeretin) can block a transporter called P-glycoprotein, which normally helps shuttle drugs in and out of your body. Verapamil rides on this transporter, so blocking it could let a bit more verapamil get absorbed and build up in your blood.

If levels climbed, you might notice more of verapamil's usual effects, such as low blood pressure, dizziness, a slower heart rate, tiredness, or constipation. That said, this caution comes only from lab dish studies, not from people, so it's theoretical. Everyday citrus in food almost certainly won't matter; concentrated supplements are the bigger question. If you want to use them, run it by your pharmacist or doctor first.

Verapamil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence suggests that methoxylated flavones might inhibit cytochrome P450 3A4 (CYP3A4). This effect seems to be structure-dependent, and does not occur with all methoxylated flavones. So far this interaction has not been reported in humans. Theoretically, concurrent use of certain methoxylated flavones with drugs metabolized by CYP3A4 might result in increased drug levels and an increased risk for adverse effects.

Here's the gist: your body clears verapamil using a liver enzyme called CYP3A4. What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
