Why this applies to Enasidenib

Enasidenib falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Methoxylated Flavones interacts with.

Some in vitro evidence suggests that methoxylated flavones might induce CYP1A2, possibly by increasing gene transcription. However, other in vitro research has not shown this effect. So far this interaction has not been reported in humans. Theoretically, concurrent use of methoxylated flavones and drugs metabolized by CYP1A2 might increase drug metabolism, decrease serum levels, and reduce effectiveness.
Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist for Idhifa (enasidenib), your leukemia medicine. Methoxylated flavones from citrus peel have shown, in a few lab dishes, that they might rev up an enzyme called CYP1A2, which helps your body break down certain drugs. If that happened with enasidenib, your body could clear it a little faster, leaving less in your blood and potentially weakening how well it fights the cancer. Since enasidenib works on its own (it isn't a prodrug needing activation), faster clearance simply means less benefit, not more.

The honest truth: this is only a theoretical concern. It hasn't been seen in real people, and other lab studies didn't show the effect at all. Still, with a cancer drug this important, please run any citrus-peel supplement past your oncologist or pharmacist before starting it. Eating normal amounts of citrus is not the worry here.

Enasidenib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence suggests that methoxylated flavones might inhibit cytochrome P450 3A4 (CYP3A4). This effect seems to be structure-dependent, and does not occur with all methoxylated flavones. So far this interaction has not been reported in humans. Theoretically, concurrent use of certain methoxylated flavones with drugs metabolized by CYP3A4 might result in increased drug levels and an increased risk for adverse effects.

Here's the short version: your leukemia medicine Enasidenib (Idhifa) is partly cleared by a liver enzyme called CYP3A4. Certain methoxylated flavones from citrus peel may slow that enzyme down in lab studies. If they do the same thing in your body, more Enasidenib could linger in your bloodstream, which in theory could mean stronger effects and a higher chance of side effects like nausea, low appetite, elevated bilirubin, or a serious reaction called differentiation syndrome.

The important part: this is theoretical, based only on test-tube work, and has never actually been reported in people. Enasidenib is the active drug itself, not a prodrug, so blocking this enzyme would push levels up, not down. Concentrated supplements are the concern, not a little citrus in food. Please run any supplement past your oncology team first.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
