Why this applies to Ibuprofen, Hydrocodone

Ibuprofen, Hydrocodone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Methoxylated Flavones interacts with.

In vitro evidence suggests that methoxylated flavones might inhibit cytochrome P450 3A4 (CYP3A4). This effect seems to be structure-dependent, and does not occur with all methoxylated flavones. So far this interaction has not been reported in humans. Theoretically, concurrent use of certain methoxylated flavones with drugs metabolized by CYP3A4 might result in increased drug levels and an increased risk for adverse effects.
Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist for your ibuprofen and hydrocodone combo (sold as Ibucodone). The hydrocodone half is partly handled by a liver enzyme called CYP3A4. In test-tube studies, certain methoxylated flavones from citrus peel may slow that enzyme down a bit. If that happened in real life, hydrocodone could hang around longer and feel a little stronger, which might mean more drowsiness, dizziness, constipation, or slowed breathing.

But here's the important part: this is only lab data, and it hasn't actually been seen in people. Not every methoxylated flavone even does this. So I wouldn't lose sleep over it. A normal citrus-flavored snack is no concern, but if you're using concentrated supplements, just mention it to your pharmacist or doctor first.

In vitro evidence suggests that some methoxylated flavones have antiplatelet effects. Theoretically, methoxylated flavones might additive effects when used with anticoagulant or antiplatelet drugs.
Some anticoagulant or antiplatelet drugs include aspirin, clopidogrel (Plavix), diclofenac (Voltaren, Cataflam, others), ibuprofen (Advil, Motrin, others), naproxen (Anaprox, Naprosyn, others), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, warfarin (Coumadin), and others.

Here's the short version for this combo pill (which mixes the painkiller ibuprofen with the opioid hydrocodone). The concern is really about the ibuprofen side. Ibuprofen is an NSAID, and like aspirin, it can make your platelets a little less sticky, which slows blood clotting. In lab dishes, these citrus-peel flavones also showed a mild platelet-slowing effect. Put them together and, in theory, you could nudge your bleeding risk up a bit.

I'll be honest: this is theoretical, based on test-tube data, not real patients, so I wouldn't lose sleep over it. Still, watch for easy bruising, nosebleeds, or stomach upset and dark stools. If you take these regularly, just check with your pharmacist or doctor first.
