Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist for Tramacet (tramadol plus acetaminophen). The concern is that methoxylated flavones from citrus peel might speed up an enzyme called CYP1A2, which could clear certain medicines a bit faster. In theory, that means slightly lower drug levels and a weaker effect. For the tramadol piece, that might translate to less pain relief than you expect.
Honestly though, this is very shaky ground. It comes only from lab dish studies, some of which didn't even show the effect, and it has never been reported in real people. CYP1A2 also isn't the main pathway for either tramadol or acetaminophen, so I wouldn't lose sleep over it. Casual citrus is a non-issue, but if you want to use concentrated supplements, run it by your pharmacist first.

Here's the wrinkle with this combo. Tramadol is partly a prodrug, meaning your body has to convert it into its stronger pain-relieving form. The enzyme CYP3A4 is one of the players that handles tramadol. In test-tube studies, certain methoxylated flavones from citrus peel may slow CYP3A4 down a bit. If that happened in people, tramadol could shift around in your system, which might mean more drowsiness, dizziness, nausea, or constipation, or it could nudge things the other way and change how much pain relief you get.
Honestly, this is only lab-level evidence and has never been reported in real patients, so I wouldn't lose sleep over it. The acetaminophen half isn't the concern here. Still, if you plan to take a concentrated supplement regularly, run it by your pharmacist or doctor first.
