Safinamide Mesylate falls under Serotonergic Drugs — the group Methylene Blue interacts with.

Combining serotonergic drugs with methylene blue can cause additive serotonergic effects, including serotonin syndrome. Methylene blue inhibits monoamine oxidase A. Intravenous or intrapulmonary use has resulted in serotonin syndrome, including reported fatalities, in patients taking some serotonergic drugs. This effect has not been reported with oral methylene blue. However, tell patients to avoid taking methylene blue with serotonergic drugs.

Here's the concern with these two together. Your Xadago (safinamide) is an MAO-B inhibitor that raises dopamine and also nudges serotonin activity up. Methylene blue is itself a strong blocker of monoamine oxidase A, another enzyme that helps clear serotonin. Stack them and serotonin can climb too high, which can trigger serotonin syndrome. Think agitation, sweating, a racing heart, tremor, muscle twitching, high fever, and confusion.

The most serious cases (including fatalities) came from methylene blue given by IV or into the lungs, not the oral or supplement form, so the real-world risk from a capsule is less clear. Still, this combo is worth avoiding. Please loop in your pharmacist or neurologist before pairing them, and don't stop your Parkinson's medicine on your own.
