Oral treprostinil (Remodulin) is used for pulmonary arterial hypertension, and one of its built-in effects is widening blood vessels, which can lower your blood pressure. Nattokinase may gently nudge blood pressure down too. So the concern here is simple addition: stacking two things that can lower pressure could leave you feeling lightheaded, dizzy, flushed, or even faint, especially when you stand up quickly.

I want to be honest that this is a theoretical caution based on how each works, not strong proof. Treprostinil also has a meaningful clotting and bleeding angle, and nattokinase can thin the blood, which is another reason to be careful. Please don't start or stop anything on your own. Check with your pharmacist or doctor first.

Here's the thing about taking nattokinase alongside oral treprostinil (Remodulin): treprostinil itself isn't a blood thinner in the classic sense, but it's a prostacyclin that relaxes blood vessels and also makes platelets less sticky. So it already nudges your blood toward staying less clotted. Stack a fibrinolytic like nattokinase on top, and you may add to that effect, which could raise your chance of bruising or bleeding.

To be honest, this concern is mostly theoretical, drawn from lab studies plus one case report involving aspirin, not strong human data with treprostinil. Still, since you're managing pulmonary hypertension, I'd play it safe. Please don't start or stop anything on your own. Just run the nattokinase by your pharmacist or doctor first.

About Treprostinil

What Is Treprostinil Used For?

Brand names: Remodulin, Orenitram

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
