Adagrasib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the situation with Krazati (adagrasib), a targeted cancer pill used for certain lung cancers with a KRAS G12C mutation. Your body clears it using a liver enzyme called CYP3A4. Phellodendron contains berberine, which lab and early human studies suggest can partially block that same enzyme. If it slows the breakdown of adagrasib, more drug could build up in your blood.

Adagrasib is not a prodrug, so higher levels would mean stronger effects and more side effects, things like nausea, diarrhea, fatigue, liver enzyme changes, or heart rhythm (QT) effects that already need watching. This is theoretical for now, based on how berberine behaves, not proven head-to-head. Still, with a cancer drug like this, even small shifts matter. Please check with your oncology team or pharmacist before combining them.

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. About Adagrasib

What Is Adagrasib Used For?

Brand name: Krazati

Krazati (adagrasib) is used to treat adults with locally advanced or metastatic non-small cell lung cancer (NSCLC) whose tumors carry a specific genetic change called a KRAS G12C mutation and who have already received at least one prior cancer treatment. Tumor testing with an FDA-approved test is required before starting treatment.

Krazati is also used — in combination with the drug cetuximab — to treat adults with locally advanced or metastatic colorectal cancer (CRC) that has the KRAS G12C mutation and has already been treated with specific chemotherapy regimens. Both uses were approved under the FDA's accelerated approval pathway based on tumor response data; continued approval depends on confirmatory trial results.
