Voriconazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2C9.
Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, phellodendron might also inhibit CYP2C9.

Here's the gist: voriconazole is partly cleared by a liver enzyme called CYP2C9. Phellodendron (through its berberine content) may slow that enzyme down a bit. If it does, your body could clear voriconazole a little more slowly, letting levels creep up. Voriconazole is an active drug (not a prodrug), so higher levels generally mean stronger effects and a greater chance of side effects like visual disturbances (blurry or bright vision), nausea, headache, or liver irritation.

That said, this is a theoretical concern based on how these compounds behave, not on real-world reports with voriconazole. Voriconazole already needs careful dosing, so I wouldn't combine it with phellodendron without checking first. Please loop in your pharmacist or doctor before starting this supplement.

Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist for Vfend (voriconazole): your liver uses an enzyme called CYP3A4 to help break this antifungal down. Phellodendron contains berberine, which in lab and early human studies appears to mildly slow that same enzyme. So in theory, pairing the two could let voriconazole build up a bit more than expected in your bloodstream.

If levels climb, you might notice more of voriconazole's known effects, like visual changes (blurry or bright vision), nausea, or higher liver enzymes. To be clear, this concern is theoretical and based on how the chemistry should work, not on solid real-world reports. Still, voriconazole is a drug doctors often monitor closely, so I'd check with your pharmacist or prescriber before combining them rather than guessing.

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. Talk with your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.

About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
