Zanubrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Your body clears zanubrutinib (Brukinsa) mainly through a liver enzyme called CYP3A4. Phellodendron contains berberine, which lab tests and some early human studies suggest can slow that same enzyme down. If the enzyme works more slowly, this cancer medicine could clear out less efficiently and build up a bit higher in your blood. That could mean stronger effects and a greater chance of side effects like bruising or bleeding, infections, low blood counts, tiredness, or an irregular heartbeat.

I want to be honest here: this is a theoretical concern based on how these compounds behave, not proven harm in people taking them together. Still, because zanubrutinib is a serious cancer drug, please check with your oncologist or pharmacist before adding phellodendron rather than deciding on your own.

What Is Zanubrutinib Used For?

Brand name: Brukinsa

Brukinsa (zanubrutinib) is used to treat adult patients with several blood cancers. These include chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma (SLL), Waldenström's macroglobulinemia (WM), mantle cell lymphoma (MCL) after at least one prior treatment, relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 regimen, and relapsed or refractory follicular lymphoma (FL) in combination with obinutuzumab after two or more prior lines of treatment.

Some of these uses — MCL, MZL, and FL — are approved under accelerated approval, meaning they are based on tumor response rates rather than confirmed long-term survival benefit. Continued approval for those uses may depend on results from confirmatory trials.
