Cobimetinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the short version for anyone on Cotellic (cobimetinib) who is curious about Phellodendron. Your body clears cobimetinib mainly through a liver enzyme called CYP3A4. Phellodendron contains berberine, which lab and early human studies suggest can partly slow that enzyme down. If the enzyme works slower, cobimetinib could build up in your blood and hit harder than intended.

For this drug, more in your system can mean more of its known side effects, things like diarrhea, nausea, skin rash, sun sensitivity, or changes in heart and eye function that your cancer team already watches closely. Right now this is a theoretical concern based on how the enzyme behaves, not proven harm. Still, with a cancer medicine I'd play it safe. Please check with your oncologist or pharmacist before adding Phellodendron.

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. About Cobimetinib

What Is Cobimetinib Used For?

Brand name: Cotellic

Cobimetinib is approved for two uses in adults. First, it is used in combination with another drug called vemurafenib to treat melanoma (a type of skin cancer) that cannot be surgically removed or has spread to other parts of the body — but only when the tumor carries a specific genetic change called a BRAF V600E or V600K mutation. A lab test must confirm this mutation before treatment begins.

Second, cobimetinib can be used on its own (as a single agent) to treat adults with histiocytic neoplasms — a rare group of cancers involving immune cells called histiocytes.
