Acetaminophen, Caffeine, Pyrilamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist with this combo product (the acetaminophen, caffeine, and pyrilamine you'd find in something like Midol). Phellodendron contains berberine, and lab and early human studies suggest berberine can mildly slow down a liver enzyme called CYP3A4. In theory, that could let a drug cleared by that pathway hang around a little longer than usual.

Honestly, this is a theoretical concern only, based on how things work in a test tube, not real-world reports. None of these three ingredients lean heavily on CYP3A4, so a big problem is unlikely. Still, if you tend to feel jittery or wired from caffeine, watch for that. To be safe, just run it by your pharmacist before pairing them.
