Bupivacaine, Meloxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2C9.
Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, phellodendron might also inhibit CYP2C9.

The Zynrelef kit pairs bupivacaine (a local numbing medicine) with meloxicam (an anti-inflammatory). Meloxicam is the piece that matters here, since your liver clears it using an enzyme called CYP2C9. Phellodendron contains berberine, and early research hints that berberine can slow that same enzyme down. If it does, the theory is that meloxicam could hang around a bit longer than expected.

In plain terms, that might nudge up the usual meloxicam side effects, like stomach upset, heartburn, fluid retention, or kidney strain. I want to be honest though: this is only a theoretical concern based on lab and pharmacology data, not real-world reports. Zynrelef is also a one-time surgical dose, so the window is short. Still, mention any supplements to your surgeon or pharmacist beforehand.
