Here's the gist for your Sudafed Cold/Flu. The dextromethorphan piece (the cough suppressant) is cleared by a liver enzyme called CYP2D6. Phellodendron contains berberine, which lab studies and some early human data suggest can slow that enzyme down. If it does, dextromethorphan may not break down as quickly and could build up in your body.

In real life, that might mean more drowsiness, dizziness, or feeling foggy, and at higher levels, restlessness. Keep in mind this is theoretical, based mostly on how berberine behaves, not strong patient studies. The pseudoephedrine part isn't really affected this way. If you take both, just mention it to your pharmacist or doctor first.

Here's the simple version. The cough part of this combo, dextromethorphan, gets broken down by a liver enzyme called CYP3A4 (and also CYP2D6). Phellodendron contains berberine, which lab and early human studies suggest can mildly slow CYP3A4. If that enzyme is slowed, a little more dextromethorphan could linger in your body, which might add to side effects like drowsiness, dizziness, or feeling foggy.

I want to be honest though: this concern is theoretical, based mostly on how the enzyme behaves, not on real cases. The pseudoephedrine portion isn't cleared this way, so it's less of an issue here. Most people won't notice anything, but since this is a short-term cold remedy, run it past your pharmacist or doctor first, especially with concentrated supplement doses.

Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan. Theoretically, phellodendron may also inhibit the metabolism of dextromethorphan.
