Ibuprofen, Pseudoephedrine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2C9.
Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, phellodendron might also inhibit CYP2C9.

Here's the gist with this combo product (Dristan). The ibuprofen part is cleared by a liver enzyme called CYP2C9. Phellodendron contains berberine, which lab and early human research suggests can slow that enzyme down. If ibuprofen is broken down more slowly, a bit more can build up in your body, which could nudge up the usual ibuprofen side effects like stomach upset, heartburn, or, with regular use, higher blood pressure or kidney strain.

I want to be honest though, this is theoretical and based on how these compounds behave, not on strong real-world studies. The pseudoephedrine side isn't really affected by this. For most folks an occasional dose isn't a big worry, but check with your pharmacist or doctor before pairing them regularly.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Here's the gist with your Dristan, which combines ibuprofen (an anti-inflammatory pain reliever) and pseudoephedrine (a decongestant). The ibuprofen part is the piece that matters here. Ibuprofen already makes platelets a little less sticky, so it can nudge up bleeding risk on its own. Phellodendron contains berberine, which in lab and animal studies also seems to slow platelet clumping. Put the two together and, in theory, you could see a bit more bruising, nosebleeds, or stomach upset and bleeding.

I want to be honest about the evidence: this is theoretical, based on test-tube and animal work, not solid human studies. For occasional cold-relief use it's likely minor, but if you take blood thinners, have stomach ulcers, or use phellodendron regularly, please check with your pharmacist or doctor first.
