Paclitaxel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the concern with pairing this herb and your chemotherapy. Your body relies on a liver enzyme called CYP3A4 to break down and clear paclitaxel. Phellodendron contains berberine, which lab and early human studies suggest can moderately slow that same enzyme down. If clearance slows, more paclitaxel could linger in your system than your care team planned for.

With this drug, higher levels can mean worse side effects: things like low blood counts (raising infection and bleeding risk), nerve tingling or numbness, mouth sores, and more fatigue. I'll be honest, this is a theoretical interaction based on how the herb behaves, not something proven in cancer patients. Still, chemo is dosed very carefully, so please don't add this supplement without first clearing it with your oncologist or pharmacist.

What Is Paclitaxel (with albumin) Injection Used For?

Brand names: Taxol, Abraxane

Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound) is used to treat three types of cancer. It treats metastatic breast cancer in patients whose prior chemotherapy has stopped working or who relapsed within 6 months of completing adjuvant (post-surgery) chemotherapy. It is also used as a first-line treatment for locally advanced or metastatic non-small cell lung cancer (NSCLC) — given together with carboplatin — in patients who cannot have surgery or radiation therapy.

Additionally, it is approved as a first-line treatment for metastatic adenocarcinoma of the pancreas, given in combination with gemcitabine. All three uses apply specifically to Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound), including the brands Abraxane and related albumin-bound formulations.
