Here's the simple version: pseudoephedrine (Efidac 24) is itself a stimulant-type decongestant. It already nudges up your heart rate and blood pressure and tightens blood vessels. Phenpromethamine is a hidden stimulant sometimes slipped illegally into supplements, and it works in a similar way. Stacking two stimulants together can add up, so the worry is too much of the same effect at once.

In practice that could mean a racing or pounding heart, higher blood pressure, jitteriness, headache, or trouble sleeping. This is a theoretical concern based on how these drugs behave, not proven by studies, but it's a real reason to be careful. Pseudoephedrine is not a prodrug, so there's no flip here. My honest advice: avoid any product listing phenpromethamine, and check with your pharmacist first.
