Here's the simple version. The brand product Miochol-E is acetylcholine that an eye surgeon places directly into your eye during surgery to make the pupil get small quickly. It's a cholinergic drug, meaning it stimulates the same pathways Poria mushroom might gently nudge in the same direction. In lab and animal studies, Poria has slowed down acetylcholinesterase, the enzyme that breaks acetylcholine down. In theory, less breakdown could mean a slightly stronger or longer cholinergic effect.

In real life, this is very unlikely to be a problem. This medicine is used once, briefly, by a doctor in a controlled setting, and the interaction has only been seen in animals, not people. Still, it's smart to tell your eye surgeon about any supplements you take.

Poria mushroom (Fu Ling) is a fungus long used in Traditional Chinese Medicine, mainly as a mild diuretic and digestive and calming aid. Modern scientific evidence in humans is very limited, so its benefits are not well proven. It is generally considered well tolerated, but talk with your pharmacist or doctor before using it, and avoid it in pregnancy and breastfeeding due to a lack of safety data.
