Here's the gist for Pimozide (Orap): this medicine has some anticholinergic activity, meaning part of how it acts is by quieting the chemical messenger acetylcholine. In animal studies, poria mushroom appears to do the opposite by keeping more acetylcholine around. In theory, that could slightly push back against pimozide's effects, possibly making it a touch less effective at controlling tics.

I want to be honest with you, though: this is only a theoretical concern from lab and animal work, and it has never been shown to happen in real people. Pimozide is not a prodrug, so this isn't about changing blood levels. I wouldn't lose sleep over it, but since pimozide is a serious medication, please run any poria use by your pharmacist or doctor first.

Poria mushroom (Fu Ling) is a fungus long used in Traditional Chinese Medicine, mainly as a mild diuretic and digestive and calming aid. Modern scientific evidence in humans is very limited, so its benefits are not well proven. It is generally considered well tolerated, but talk with your pharmacist or doctor before using it, and avoid it in pregnancy and breastfeeding due to a lack of safety data.

What Is Pimozide Used For?

Brand name: Orap

Pimozide tablets are used to suppress motor tics (involuntary movements) and phonic tics (involuntary sounds or words) in people with Tourette's Disorder. It is reserved for patients whose daily life or development is severely affected by their tics — it is not intended as a first-line treatment, and it is not used for tics that are mild or mainly cosmetic concerns.

Pimozide is not meant for tics caused by other conditions or medications, and it should not be used for simple tics unrelated to Tourette's Disorder.
