Why this applies to Esomeprazole

Esomeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2C19.
In vitro research shows that propolis extract can inhibit CYP2C19. However, animal research shows that propolis extract does not significantly affect CYP2C19 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C19 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist for esomeprazole (Nexium): your liver uses an enzyme called CYP2C19 to clear it from your body. In a test tube, propolis seems to slow that same enzyme down. If that happened in real life, esomeprazole could linger a bit longer and its levels could creep up, which in theory might mean slightly more of its usual effects, like headache, nausea, or an upset stomach.

The reassuring part is that this is very weak evidence. It comes from lab dishes only, and animal studies didn't show a real effect, likely because not much propolis actually reaches the bloodstream. It hasn't been reported in people. Still, if you take concentrated propolis supplements daily, it's worth a quick chat with your pharmacist.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist with propolis and esomeprazole (Nexium). Your body clears esomeprazole partly through a liver enzyme called CYP3A4. In a test tube, some propolis compounds appear to slow that enzyme down, which in theory could let a bit more esomeprazole build up. Esomeprazole isn't a prodrug, so more drug would mean slightly stronger effects and a small bump in side effects like headache, nausea, gas, or loose stools.

The reassuring part: this is only lab-and-animal data, and it has never actually been seen in people. Animal studies didn't show a real effect, likely because so little propolis reaches the bloodstream. Everyday or occasional propolis use is unlikely to matter, but if you take high-dose supplements daily, mention it to your pharmacist or doctor.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. What Is Esomeprazole Used For?

Brand name: Nexium

Esomeprazole treats problems caused by excess stomach acid. It's used to heal erosive esophagitis (damage to the esophagus from acid), control GERD (gastroesophageal reflux disease) symptoms like heartburn, and protect the stomach lining in people at risk for ulcers caused by NSAIDs (anti-inflammatory pain relievers like ibuprofen).

It's also used as part of a three-drug combination to wipe out H. pylori — a stomach bacteria linked to ulcers — and for people with rare conditions that cause the stomach to overproduce acid, such as Zollinger-Ellison syndrome.
