Esomeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C19 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.

Here's the gist with Nexium (esomeprazole) and adrafinil. Your body turns adrafinil into modafinil, and in lab studies modafinil can slow down a liver enzyme called CYP2C19. That same enzyme helps clear esomeprazole from your system, so in theory esomeprazole could build up a bit higher than usual.

The good news is esomeprazole is a pretty forgiving medication, so even a modest increase isn't likely to cause anything dramatic. You might just notice more of its usual mild effects, like headache, nausea, or stomach upset. I'll be honest though, this interaction is theoretical and based only on test-tube data, not real patients. Still, since adrafinil is unregulated and hard on the liver, please run it by your doctor or pharmacist before combining them.

Here's the gist for Nexium (esomeprazole): after your liver turns adrafinil into modafinil, that modafinil can gently speed up an enzyme called CYP3A4. Since esomeprazole is partly cleared by this same enzyme, revving it up could clear the drug a little faster and slightly soften how well it controls your stomach acid. In real life, that might show up as heartburn or reflux symptoms creeping back.

I want to be honest, though: this is a theoretical concern based on how the drugs work, not something proven in studies, and modafinil is only a weak inducer. So the effect, if any, is likely small. Before combining these, please run it by your pharmacist or doctor.

What Is Esomeprazole Used For?

Brand name: Nexium

Esomeprazole treats problems caused by excess stomach acid. It's used to heal erosive esophagitis (damage to the esophagus from acid), control GERD (gastroesophageal reflux disease) symptoms like heartburn, and protect the stomach lining in people at risk for ulcers caused by NSAIDs (anti-inflammatory pain relievers like ibuprofen).

It's also used as part of a three-drug combination to wipe out H. pylori — a stomach bacteria linked to ulcers — and for people with rare conditions that cause the stomach to overproduce acid, such as Zollinger-Ellison syndrome.
