Quercetin and Afatinib Dimaleate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Afatinib Dimaleate

Afatinib Dimaleate falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Gilotrif (afatinib). This cancer pill leans heavily on a gut transporter called P-glycoprotein, which acts like a bouncer that pumps some of the drug back out of your system. Quercetin in high-dose supplement form may slow that pump down. If less afatinib gets pumped out, more can be absorbed, which could nudge your blood levels up and make this drug's typical side effects worse, things like diarrhea, mouth sores, dry or acneform skin rash, and sore, cracked skin around the nails.

I want to be honest about the evidence: this is mostly theoretical and based on small, mixed studies with other drugs, not afatinib itself. Food amounts of quercetin (apples, onions, tea) are very unlikely to matter. Before adding a concentrated supplement, please run it by your oncology team or pharmacist.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

About Quercetin

What is Quercetin?

Quercetin
  • Seasonal allergies
  • Antioxidant support
  • Heart and blood pressure health
  • Anti-inflammatory support
  • Immune support
  • Exercise recovery

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before taking high-dose supplements.

About Afatinib

What Is Afatinib Used For?

Brand name: Gilotrif

Gilotrif (afatinib) is used to treat metastatic non-small cell lung cancer (NSCLC) — cancer that has spread and cannot be removed by surgery. It has two specific approved uses.

First, Gilotrif is used as an initial treatment for metastatic NSCLC when the tumor carries certain EGFR mutations (gene changes in the cancer cells) confirmed by an FDA-approved test — specifically non-resistant mutations. Second, Gilotrif is used to treat metastatic squamous NSCLC (a particular cell type of lung cancer) in patients whose cancer progressed during or after platinum-based chemotherapy.
